PART I
Title of Activity: SCHOOL BASED FEEDING PROGRAM (SBFP)
Date: December 17- 27, 2020 (First Trance)
January 25-June 3, 2021 (Second Trance)
Venue: San Antonio Central Elementary School
Budget Allocation: Php 15,300.00 (Maintenance and Operating Expenses)
Source of Income:
Objectives:
1. to rehabilitate the increasing number of children identified with high
malnourished cases from kindergarten to grade 6 during COVID 19
pandemic,
2. to improve the children’s health and nutrition values and behavior,
3. to promote good health and eradicating malnutrition, and
4. to encourage learners to enroll amidst COVID 19 pandemic.
Expected Number of Participants: 110 kindergarten, 36 primary beneficiaries, 7
Secondary beneficiaries, 7 teachers, 2 non-
2 teaching personnel, 1 School Head

PART 2
NARRATIVE REPORT
1. Introduction/ Rationale/ Objectives
As guided by several department orders issued by DepEd, the School-
Based Feeding Program (SBFP) was implemented amidst the COVID 19 pandemic.
This program aims to address hunger and encourage learners to enroll, contribute
to the improvement of health and nutritional status of public-school learners,
enhance, and improve their health and nutrition values and help boost their
immune system.
The target beneficiaries of the feeding program are all learners from
incoming kindergarten, the primary beneficiaries from grades 1-6 who were
wasted and severely wasted based on the SY 2019-2020 SBFP report. Due to the
pandemic, secondary beneficiaries are included which are the 4P’s beneficiaries.
2. Narration of Accomplishment
Before the Feeding Program
Due to COVID 19 pandemic, social and economic activities are
restricted. DepEd declared that no pupils will go to school, instead modular and
online learning will be the new mode of learning for SY 2020-2021 in accordance
with government policies such as social distancing. Despite this, DepEd announced
the implementation of School-Based Feeding Program in schools.
During the Feeding Program
The first trance was on December 17-27, 2020. Milk was given to 36
primary beneficiaries and 4 secondary beneficiaries from grades 1-6. The parents
or guardians of the learners were requested to claim the milk upon picking up of
modules.
The second trance consisted of pasteurized milk, fruits, and bread
were given from January 25, 2021-June 3, 2021 to learners in kindergarten (110
pupils), grade 1 (5 pupils), grade 2 (6 pupils), grade 3 (13 pupils), grade 4 (1 pupil),
grade 5 (9 pupils), grade 6 (2 pupils), and 4P’S (7 pupils), a total of 153 pupils.
After the Feeding Program
At the end of the feeding program, Mrs. Cynthia F. De Asis conducted
a survey to find out whether the food gave significant effects in the overall health
and well-being of the beneficiaries. Mrs. De Asis made phone calls to parents or
guardians, and almost all reported that their child started gaining weight and
having more enthusiasm in their studies. Some parents even requested to let their
child be part of the feeding program again next school year.
With the help of the newly installed School Head, Mdm. Liza L. Badillo
and all the members of the feeding team, Mrs. De Asis, accomplished all the
necessary forms and the terminal report.
In the terminal report, it was also indicated that 100% of the
beneficiaries were dewormed. Their attendance during the feeding program was
100%. The budget allocation of PHP 15,300.00 was fully liquidated.
