It is difficult to understand the development of autogenic training in Spain without realizing how devastated Spanish Psychiatry was after the Civil War. Most of its prominent practitioners were forced into exile. Mira-Lopez, the inventor of the myokinetic test, went to England. Angel Garma went to Argentina, where he soon became the president of Buenos Aires Psychoanalytic Association. Miguel Prados went to Canada, were he co-founded McGill University Psychiatry Department. Tosquelles went to France, were he was soon the director of a most progresist psychiatric hospital. De Ajuriaguerra became University Professor and the Chairman of Geneve’s celebrated Psychiatric Services... Spanish Psychiatry became stagnant for decades, but Spain’s loss became the World’s gain. When Schultz’s Autogenes Training was first translated into Spanish in 1954 (1), there were few high ranking professionals available to take the lead.

Psychological phenomena secondary to bronchial asthma play a relevant role in this disease influencing in a remarkable way its clinical course in each patient. Thus, an appropriate knowledge of the reactive and adjustment phenomena may lead to an outstanding medical outcome in response to the relevant conventional medical treatments usually carried out in these patients. In our present paper diverse psychosological variables involving psychological responders to a chronic disease as bronchial asthma are analysed. Such variables include so important topics as "illness behaviour", "life style", coping style" and treatment compliance in this very clinical condition.

Objective:  To evaluate the long‐term stability of International Classification of Diseases‐10th revision bipolar affective disorder (BD) in multiple settings.Method:  A total of 34 368 patients received psychiatric care in the catchment area of a Spanish hospital (1992–2004). The analyzed sample included patients aged ≥18 years who were assessed on ≥10 occasions and received a diagnosis of BD at least once (n = 1153; 71 543 assessments). Prospective and retrospective consistencies and the proportion of subjects who received a BD diagnosis in ≥75% of assessments were calculated. Factors related to diagnostic shift were analyzed with traditional statistical methods and Markov's models.Results:  Thirty per cent of patients received a BD diagnosis in the first assessment and 38% in the last assessment. Stress, unpleasant emotions and autonomic imbalance may play a main role in precipitating asthmatic attacks. In this study two homogeneous groups of asthmatic patients (N = 24) are treated over an eight-month period. The experimental group was treated with autogenic therapy and the control group with supportive group psychotherapy. Respiratory function parameters measured were Forced Vital Capacity (FVC), Forced Expiratory Volume in the first sec (FEV I), Forced Expiratory Flow between 25% and 75% of the FVC (FEF25-75%). and Mesoexpiratory Flow (MEF50%). The group under Autogenic Therapy obtained a relevant clinical improvement (> 15% of pretreatment values) in respiratory function. No significant changes were observed in the control group. These results suggest that autogenic therapy could be an effective adjunctive treatment in bronchial asthma.

In this paper the features, pattern profile and frequency of psychiatric and psychosomatic symptomatology of a sample constituted by 40 medical students during the usual academical period--free fron examination--are analyzed. Results suggest that there are no significant differences between the symptomatic patterns of the students of our sample and those belonging to the general population in our context. The Possible role played by stress associated to "examination" in reaching a high level of psychopathology in these students is discussed.

Ecpathy is a new concept, complementary of empathy, that allows appropriate dealing of induced feelings and emotional contagion. Empathy is “the action of understanding, being aware of, being  sensitive to, and vicariously experiencing the feelings, thoughts, and experience of another of either the past or present without having the feelings, thoughts, and experience fully communicated in an objectively explicit manner“ (Webster). As such, empathy is considered a most useful tool in psychology and medicine. The capacity of being empathic is regarded as a basic ability for human understanding. The converse of empathy is Ecpathy, a volontary mental process of actively excluding feelings, attitudes, thoughts and motivations induced by others. The development of ecpathic abilities is most useful for dealing with projective identification, both in clinical and extraclinical situations, and for preventing the influence of emotional manipulation and emotional contagion.

Integrative Sequential Psychotherapy (ISP) is a new way to integrate psychotherapeutic interventions from different orientations. The therapeutic process reproduces in the clinical setting the natural process of crisis management by resilient individuals. There are seven phases, named after the central task of each one: Owning oneself, Keep calm, Harm minimization, Understand the situation, Deciding the condition, Action no reaction and Creating new reality. In this article the three first are described. The integrative axis of PSI is the sequence of tasks central to each phase. An approach useful for one task may be irrelevant or even counterproductive in another. Surmounting one phase gets the patient ready to work in the next one; conversely, difficulty in the working of one phase indicates that he has to return to the previous one. The entire sequence has to be repeated several times, until it is fully internalized and the patient is able to follow the whole process with ease in every adverse circumstance.

Mental health is one of the priorities of the European Commission. Studies of the use and cost of mental health facilities are needed in order to improve the planning and efficiey of mental health resources. We analyze the patterns of mental health service use in multiple clinical settings to identify factors associated with high cost. 22,859 patients received psychiatric care in the catchment area of a Spanish hospital (2000-2004). They had 365,262 psychiatric consultations in multiple settings. Two groups were selected that generated 80% of total costs: the medium cost group (N = 4,212; 50% of costs), and the high cost group (N = 236; 30% of costs). Statistical analyses were performed using univariate and multivariate techniques. Significant variables in univariate analyses were introduced as independent variables in a logistic regression analysis using "high cost" (>7,263$) as dependent variable. Costs were not evenly distributed throughout the sample. 19.4% of patients generated 80% of costs. The variables associated with high cost were: age group 1 (0-14 years) at the first evaluation, permanent disability, and ICD-10 diagnoses: Organic, including symptomatic, mental disorders; Mental and behavioural disorders due to psychoactive substance use; Schizophrenia, schizotypal and delusional disorders; Behavioural syndromes associated with physiological disturbances and physical factors; External causes of morbidity and mortality; and Factors influencing health status and contact with health services. Mental healthcare costs were not evenly distributed throughout the patient population. The highest costs are associated with early onset of the mental disorder, permanent disability, organic mental disorders, substance-related disorders, psychotic disorders, and external factors that influence the health status and contact with health services or cause morbidity and mortality. Variables related to psychiatric diagnoses and sociodemographic factors have influence on the cost of mental healthcare.

Studies have shown an association between the T102C polymorphism of the 5HT2a receptor gene and schizophrenia. However, negative findings have also been reported. We conducted a case-control study of the T102C polymorphism in Spanish Caucasians. We compared T102C polymorphism genotypes and allele frequencies in 188 schizophrenia patients and 440 healthy controls. There were significant differences in the distribution of the three genotypes (TT, TC and CC) and in the allele frequencies in controls and schizophrenics. The C allele was more frequent in schizophrenia patients than in healthy controls. The Cochrane-Armitage test for trend indicated a significant dosage effect for schizophrenia of the risk allele (C).

SUMMARYA single fasting level of serum prolactin was measured in each of sixty control subjects and eighty‐three psychiatric patients of both sexes who had been on neuroleptic therapy for 2‐4 weeks (acute treatment) or at least 5 years (chronic treatment) and who were aged either 17‐45 or 48‐85 years. All groups of patients had significantly higher mean prolactin levels than controls. Gender, age group of women, and exposure to acute or chronic treatment were significant variables determining the magnitude of neuroleptic‐induced elevation of prolactin. In some of the groups, dose, duration of chronic therapy, and concomitant administration of anticholinergic drugs also influenced prolactin levels.Whereas all acutely treated women had prolactin values above the control range, one out of twelve (8.3%) of the women aged 17‐45 years and six out of fourteen (42.9%) of the women aged 48‐85 years who were under chronic treatment had normal values. This study aims to further evaluate the controversial association between the Ser9Gly polymorphism in codon 9 of the D3 dopamine receptor gene (DRD3) and schizophrenia in psychiatric inpatients acutely hospitalized in two general hospitals in Madrid, Spain. The Ser9Gly polymorphism of the DRD3 was examined in 178 schizophrenic patients, 286 patients with other psychiatric diagnoses, and 132 controls recruited. Genotype frequencies were in Hardy‐Weinberg equilibrium. No association was found between schizophrenia and the Ser9Gly polymorphism of the D3 dopamine receptor gene. © 2006 Wiley‐Liss, Inc.

L'expérience directe du corps conduit à la connaissance et à l'acceptation du «soi» physique, première étape vers la connaissance et l'acceptation de notre totalité. Au-delà de la méthode classique du Training autogène la méditation somatosensorielle prend soin de se concentrer sur l'expérience sensorielle vécue au présent et pas sur la suggestion de 1'expérience. Il s'agit donc ici de profiter du principe A.S.C.I. et non pas du principe idéoplastique. Le nombre d'exercices est augmenté a dix, au cours desquels la concentration se place successivement sur les expériences de masse, vécu comme poids, la thermogenèse, le battement du coeur et l'automatisme respiratoire. Fear of illness, confinement, lack of financial resources, or poor social support can influence people’s mental state. Despite these risks, several psychological resources may help address this situation. The present study investigated the effectiveness of a self-relaxation techniques known as autogenic training. Autogenic training is a well-known method in Europe for the treatment of anxiety and stress disorders. The practice of autogenic training is also reported to improve neurovegetative and immune regulation. This study focuses on describing how autogenic training is helping its practitioners to cope with the pandemic. There is no precise documentation about an universal nuclear composition of the depressive syndrome. Moreover, secondary manifestations of depression are even more controversial. In this paper the symptomatic profile of a sample of depressive patients attended in the Hospital Universitario de Canarias is analyzed. The characteristics, profiles and frequencies of the symptomatic pattern are discussed and commented.

Los trastornos del sueño son extraordinariamente
frecuentes en los trastornos afectivos y, aunque en general
se han considerado secundarios al trastorno afectivo
en si, cabe preguntarse si en ocasiones se trata de un
substrato común que produce ambos o incluso si es el
trastorno del sueño el que produce la psicopatología
afectiva. En el presente trabajo estudiamos el insomnio
como queja subjetiva en una muestra constituida por 128
pacientes psiquiátricos ambulatorios consecutivos con
diagnósticos incluidos en las categorías F3 (trastornos de
humor; n = 58: edad media = 39,77 [ ± 13,281] y F4
(trastornos neuróticos; n = 70; edad media = 32,37 [ ±
12,541] del CIE-10 mediante el análisis de los items 44
(dificultad para conciliar el sueño), 64 (despertares de
madrugada) y 66 (sueño inquieto o perturbado) del
inventario autoadministrado SCL-90-R. Los pacientes
pertenecientes a la categoría diagnóstica F3 obtienen
valores significativamente mayores (p < 0,05), en los tres
items, que los correspondientes a pacientes incluidos en
la categoría diagnóstica F4, y esto ocurre fundamentalmente
a expensas de las mujeres de ambas categorías diagnósticas.
Ninguno de los items experimentó correlaciones
significativas con la edad en las categorías de la
muestra, hecho éste, que contrasta con los resultados
correspondientes a lo que ocurre en la población general
del mismo medio.

SUMMARY
Sleep Disorders are particularly frequent in affective
disorders. Although they have been considered as
secondary to the existing affective disorder, we can think
whether there is a common basis that generates both
phenomena. Moreover, it could be thought that sleep
disorders could induced the very affective disorder. In
this study, insomnia subjective complaints in a sample of
128 psychiatric consecutive outpatients with F3 category
diagnosis (n = 58; mean age = 39,77 [ ± 13,28]) and F4
category diagnosis (n = 70; mean age = 32.54 [ ± 12,54])
are considered. Items 44, 64 and 66 of Derogatis SCL-
90-R Self-report questionnaire, that have to do with
insomnia complaints, are analysed. F3 diagnostic category
patients manifest significant higher values (p < 0,05)
than the patients from the F4 category diagnosis in each
of the three items considered. Mainly women are responsible
of the higher scores observed in F3 diagnostic
category patients. No significant correlations with age
was observed in any of the items studied. This fact
strongly contrasts with the results registered by general
population in our context.

Autogenic discharges are sudden and self-limited manifestations of the most varied nature, unrelated to the proprioceptive/interoceptive focus, which appear spontaneously during the practice of autogenic training. They are related to the patient's traumatic history, both psychological (stress, interpersonal conflicts, etc.) and physical (accidents, surgical interventions, intoxications, etc.). A specific discharge may occur repeatedly during the autogenic exercises, until it finally ceases to appear. As a rule, the disappearance of the discharges, especially those which have been more frequent and intense, is accompanied by clinical improvement. Luthe regarded autogenic discharges as the manifestation of a natural therapeutic process that is activated during the autogenic state, which he called "autogenic neutralization". To take advantage of this effect, he developed several advanced methods, especially indicated when the basic method is insufficient or when the discomfort produced by the discharges makes it difficult for the patient to cooperate adequately. In addition to Luthe's clinical observations, the theory of autogenic neutralization finds neurophysiological confirmation in studies on the treatment of temporal epilepsy with autogenic training.

Sin embargo, los terapeutas que poseen los siguientes rasgos tienden a obtener éxitos terapéuticos en proporción similar, independientemente del método que utilicen: Empatía objetiva, aceptación dinámica del paciente, capacidad para tolerar la angustia, libertad de la consciencia, renuncia a gratificación emocional a expensas del paciente, y motivación básica relacionada con pulsiones creativas.

The mental processes in creative activity combine primary process thinking,  as dreams and psychotic mental activity, and secondary process thinking, as in normal mature mental activity. This integration of primary and secondary processes has its own peculiar characteristics and has been termed the "tertiary process." Just as primary processes serve the pleasure principle and secondary processes the reality principle, tertiary processes are in service of individual transcendence or "transcendence principle."
Some psychotic patients can function on high creativity, both artistic and scientific. These "creative psychoses" are defined by affective and delusional psychopathology, although empathy and affective input in interpersonal relationships are maintained, and adjustment to eve1y day life is good. From a psychodynamic perspective, creative psychoses are characterized by a predominance of libidinal drives and object search and an absence of internal persecutory objects and aggressive drives. Creativity represents an attempt to repair the loss of a highly valued object, the cathexis formerly attached to the lost object being transferred to creative activity.

En 1984 Heinz Leymann elaboro una lista de 45 acciones o estrategias observadas en situaciones de acoso laboral o mobbing. Veinte años después Luis de Rivera y Manuel Abuin comprobaron en más de 600 trabajadores afectados de mobbing la realidad de estas estrategias y descubrieron 15 más. Después de varios estudios estadisticos, aplicando una valoracion tipo Likert para cada estrategia y técnicas de analisis factorial, construyeron un instrumento psicologico de medida, el Cuestionario de Estrategias de Acoso Psicologico en el Trabajo, al que pusieron el sobrenombre de LIPT-60, en honor al pionero del estudio del mobbing. La relevancia del estrés en medicina se ha hecho evidente a lo largo de tres grandes etapas históricas. La primera se inicia con los trabajos de Walter Cannon, la segunda culmina con los descubrimientos de Hans Selye y la tercera se estabiliza con la aparición de la medicina psicosomática, también llamada medicina conductual. Al primer periodo debemos la demostración del vinculo entre emociones y procesos vitales; al segundo, la popularización del término y la descripción del Síndrome General de Adaptación; al tercero, el concepto de estrés psicosocial y el descubrimiento de factores moduladores, que, como el tipo de personalidad, el locus de control o el estado afectivo, potencian, inhiben o modifican la interacción entre factores externos y factores internos de estrés. La Psicosomatica del Estres se presenta en cinco apartados: 1) La biología de las emociones,

3) El equilibrio homeostasis-alostasis y Ley General del Estres

El amor es el motor de la vida y el médico, como guardián profesional de la vida, ha de estudiar su desarrollo normal, sus vicisitudes patológicas y sus aplicaciones terapéuticas. El sentido del amor empieza a desarrollarse en el recien nacido y sigue tres fases: amor incondicional o fusion, comunicacion interactiva y amor condicional. Se describe la "Amoropenia",  déficit subjetivo de vivencia amorosa por discapacidad en percibir y generar amor. En la praxis medica, el amor regula la relacion medico enfermo. Para ser médico es necesario cumplir tres condiciones relacionadas con el amor: Primera, ser buena persona, es decir, amar la Vida. Segunda, que le guste la gente, es decir, amar en cada persona la encarnación temporal de la Vida.  Tercera, amar su profesión, es decir, estar siempre dispuesto a adquirir mayores habilidades y conocimientos en el arte de curar.
